DOCUMENTATION REQUIRED FOR REGISTRATION
In order to facilitate the registration process, you are required to you bring the original documentation from each section:
A: Eligibility
· Biometric card/Passport
and/ or Birth Certificate
B: Photo Identification
· Passport size photograph and/or Photo Driving License including the paper counterpart
C: 2 Proofs of Address
These MUST be dated within the past 2 months unless otherwise stated. The ONLY proofs of address that we can accept are as follows –
- Utility Bills. These MUST be the actual bills and NOT a circular (must be dated within the last 3 months)
- Photo Driving License including the paper counterpart
- Bank/Building Society Statement. These MUST be actual statements and NOT circulars (must be dated within the last 3 months)
- Benefit Statement (dated within the last 3 months)
- Council Tax Statement – Can be dated within the past 12 months
- Credit card Statement. This MUST be an actual statement and NOT a circular (within the last 3 months)
- Document from a government department
- Store card statement. This MUST be the actual statement and NOT a circular
- Most recent copy of your DBS if you don’t have one, we will apply for one.
D: DBS
Most recent copy of your DBS if you don’t have one, we will apply for one.
E: Evidence of Name Change
If you have changed your surname since birth you MUST be able to provide at least one of the following documents which confirms this -
- Marriage Certificate /Divorce Certificate
- Name Change Deed Poll
F:Proof of National Insurance Number
- National Insurance Card
- P45 ,P60
G: Proof of Qualifications and Training-
E.g. NVQ Certificate ,Training etc
